Community Health Programs, as a Federally Qualified Health Center, has a network of pharmacy partners, including a home delivery pharmacy. Patients have options for pharmacy and prescription refills and some pharmacies MAY offer you better pricing on your medications.

CHP is also affiliated with Equiscript, a pharmacy delivery program that provides free home delivery in pre-sorted dose packages as well as a Patient Care Advocate who will personally assist you with refills and prescription maintenance.

CHP is affiliated with the 340B Drug Discount Program, a US federal government program, created in 1992, that requires drug manufacturers to provide outpatient drugs to eligible health care organizations and covered entities at reduced prices. The intent of the program is to allow covered entities to “stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services”.
Section 340B requires pharmaceutical manufacturers to enter into an agreement, called a pharmaceutical pricing agreement (PPA), with theHHS Secretary in exchange for having their drugs covered by Medicaid and Medicare Part B. Under the PPA, the manufacturer agrees to provide front-end discounts on covered outpatient drugs purchased by specified Clinicians.
You as the patient, do not need to ask for this program, the pharmacist automatically applies the 340B discount to your medication, if eligible.
